New Hire Infomation:

The Online Background Check must be completed and cleared before you may have any contact with the clients. Faliure to do so is a violation of our contract and will effect your possibility for employment.

 

The following documents must be completed before you can receive your first pay check.

Resume

I-9 form

Direct Deposit form signed

Voided check (or bank name, routing # & account # for savings account)

2 copies of driver's license

1 copy of social security card

Copy of auto insurance

Copy of driving record

Release of information for Crim-Link background check, signed & dated

BCI filled out & signed

BCI payment policy signed

$10 for BCI

Fingerprint cards if applicable

Original mission release letter

Conflict of interest form, filled out & signed

Training documents policy

Employee Handbook completed*

Policies & Procedures manuals (each page signed)*

Code of Conduct*

Pre-service quizzes completed*

Suicide packet & incident reporting quiz*

Abe Kimball's Training

Shadow shift form completed (1 unpaid, 2 paid)

TB test (The Journey at Mount Pleasant, and True Path only)

Food Handlers permit (Phoenix, Crossroads, & New Visions only - no night staff

All of these items must be completed before you can get your first pay check.
